Filling out paperwork is one of the most unpleasant parts about visiting a medical office eastwind women’s health . There is no easy way around it, but you can now complete the forms online before visiting the office. If you have any questions, you can leave them blank. Then, you can ask the staff to clarify your doubts once you’re at the office.

You can read (and, if you wish, print and save a duplicate) our Privacy Policy

You must designate someone to access your medical information if you do not want to be the one who has access. We are committed to protecting your barn door health medical information and complying with HIPPA laws. A designation is required for spouses and other family members to be able call you for information.

  • Please fill out the Risk Assessment Questionnaire
  • Please fill out the Medical History Questionnaire

If you are a minor, and your guardian or parent is not available for your visit to us, you will need to complete the Consent Form to Medical Treatment of Minors and have it signed by them. This consent form is found under a separate tab.

  • Fill out the Request of Previous Medical Records Form and send it to us.
  • Eastwind Women’s Health: New and Returning Obstetrical Patient
  • Download the information packet, regardless of whether you’re new to our clinic or returning for a second pregnancy.

This packet includes information about our practice and useful information for you during your visit. Please take the time to review it, and don’t hesitate to ask your provider any questions. This packet serves two purposes: to help you get to know your provider and to answer any questions that may arise. You will also find references to other material.

You should not sign the Non-Invasive Prenatal Screening Form until you have had an opportunity to discuss it with your provider.
